Every call you make is private and confidential, serving as the first step for us to understand your needs, concerns, and the personalized support you're seeking. At Wellvance, our mission is to ensure timely access to services, whether through referral, individual request, or following discharge from inpatient care. We are proud to serve individuals of all ages with severe or persistent behavioral health concerns, ensuring comprehensive care for everyone.

If you prefer to talk to someone in person, you may walk in to any office location during business hours (8:00am-5:00pm, Monday-Friday). A licensed and highly-qualified Wellvance professional will be glad to meet with you and discuss your needs and potential service options.

Your Path to Care

At Wellvance, we recognize that each individual's path to health and wellness is unique. Together, we'll focus on integrating the latest research and proven methodologies to offer you a pathway toward personal growth and well-being.

Getting started with services takes 3 easy steps! First, we schedule an intake appointment. This appointment kicks-off after you contact our Access Team above and allows us to get to know each other, collect your information, discuss your history, and review your path to wellness. Next, we schedule you for a Plan of Service (if eligible). A Plan of Services is your roadmap for care; outlining your services, goals, expectations, and a review of the appointments we recommend. Afterwards, you move to your third and final step — empowering your wellness and advancing your potential! This phase is where you begin your service plan and work together with the Wellvance care staff to actively participate, attend, and achieve your goals!

The Community Electronic Health Record (CEHR) is a way you can see your Wellvance personal health information, keep track of appointments, ask questions and get answers from your Wellvance care staff.

Eligibility

Services are available to individuals who:

  • Live in Iosco, Ogemaw, and Oscoda County.
  • Have a serious mental illness, developmental disability, serious emotional disturbance, and/or co-occurring substance use disorder.
  • Have Medicaid, MiChild, or Healthy Michigan (*If you do not have insurance, we can provide you with a Medicaid application. If you do not have insurance and meet all other eligibility criteria, you may be placed on a waiting list.)
  • Demonstrate medical need for services.

If you do not qualify for Wellvance services, our staff can assist you in finding or referring other services available to you.

Payment for Services / Non-Discrimination Clause

Wellvance does not deny services based on an individual’s inability to pay for services.

Services are based on eligibility and payment is based on what you can afford. We will help you to determine your needs and conclude whether your treatment will be covered. No individual will be denied access to services due to inability to pay; and there is a discount/sliding fee schedule available based on family size and income. 

To determine your fee, a review of requested documents along with information you provide will be reviewed (proof of income is required). In general, you’ll be asked to provide information about any sources of income or health insurance that may cover services. All financial information will be held in the strictest confidence. Once a fee has been determined, you also can ask for a review of your determined fee if you do not agree with the amount you are asked to pay. If you fail to provide insurance information you may be at risk of being charged services that might otherwise be covered.

If you are enrolled in Medicaid and meet criteria for services, are below the poverty level, or possess MiChild or Healthy Michigan, and do not have a Medicaid Deductible/SpendDown account, the total cost of your authorized behavioral health and wellness services will be covered (no fees will be charged to you). If you do have a Medicaid Deductible/SpendDown account, you will be charged the sliding fee based on your family size and income up to the lessor of your monthly ability to pay or Medicaid Deductible/SpendDown amount.

Wellvance does not discriminate based on race, color, sex, national origin, disability, religion, age, sexual orientation, gender identity, inability to pay or Medicaid/Medicare CHIP status. If you feel you qualify for the sliding fee scale, or if you would like to have your service fee re-determined, please notify your care manager or Wellvance Customer Service at 989-362-8636.
